dialysis
When to start outpatient dialysis
The optimal time to start dialysis is debated. Studies are inconclusive with some favoring late initiation of dialysis either at the onset of uremic symptoms or when the estimated glomerular filtration rate (eGFR) is <10 mL/min/1.73 m2 rather than preemptive hemodialysis initiated at eGFR of 10-15 mL/min/1.73 m2 without uremic symptoms. Regardless of the timing of dialysis initiation, survival outcomes are better with renal transplantation.
New dialysis
- Usually dialyze first 3 days in a row before evaluate if can stop
Types
- CRRT: Convection
- HD: diffusion and convection
- PD: diffusion
- increase serum glucose = worse clearance because of less diffusion
Ways to combat hypotension in dialysis
- Na myelin: induce hypernatremia
- albumin infusion
- lower the temperature
Efficiency of dialysis
- measure efficiency with KT/u
- Should have drop in BUN by 65%
- Can increase efficiency with time, blood flow, dialyzer
